When Dizziness Won’t Go Away: How Habituation Helps in Vestibular Rehabilitation

We’ve all felt dizzy at some point—maybe after standing up too quickly, spinning on the playground, or even scrolling on our phones for too long. Occasional lightheadedness can be normal. But when dizziness symptoms start happening regularly, interfere with daily life, or come with imbalance and blurred vision, they may be signs of a vestibular disorder. That’s when it’s time to seek help and ask: is dizziness normal for me, or is something more going on?

At SparqPT, we specialize in physical therapy for dizziness through a tailored program called vestibular rehabilitation therapy (VRT). This exercise-based approach uses the brain’s natural ability to adapt and retrain itself, helping people move past dizziness, vertigo, and imbalance.

The Four Pillars of Vestibular Rehab

Vestibular rehab is not one-size-fits-all. Instead, treatment is built around four main strategies—or “pillars”—that address different aspects of the vestibular system:

1. Adaptation

Adaptation is all about helping your brain “reset” the way it coordinates vision and movement. When the vestibular system isn’t working properly, you may notice blurred vision when moving your head, like words on a page bouncing around when you look back and forth. Adaptation exercises train the vestibular-ocular reflex (VOR), which is the reflex that keeps your vision stable while your head moves. Over time, this retrains the brain for clearer, steadier vision.

2. Habituation

Habituation exercises focus on reducing dizziness that’s triggered by specific movements or environments. Rather than avoiding these triggers, you work with your therapist to safely and gradually reintroduce them until your brain learns they are not threatening. This decreases the “overreaction” that causes dizziness.

3. Substitution

Sometimes, parts of the vestibular system are permanently damaged. In these cases, substitution helps the brain lean more on other senses—like vision and body awareness—to maintain balance. This pillar is key for long-term stability and independence when dizziness or imbalance won’t resolve completely.

4. Cognitive Strategies

Dizziness doesn’t just affect the body—it also impacts concentration, confidence, and even anxiety levels. Cognitive strategies combine movement with mental tasks, helping people stay sharp and steady in real-world situations. Training the brain to multitask under challenging conditions is a vital part of getting back to daily life.

Together, these four pillars create a flexible, individualized framework that addresses the many different ways vestibular disorders can affect daily life.

Habituation: A Closer Look

Think about learning to ride a bike for the first time. At first, balancing feels tricky, and you may wobble or even fall. But with repeated practice, your brain and body learn to coordinate, and soon riding feels natural and effortless. Habituation works the same way in vestibular rehab: repeated, safe exposure to movements or environments that once triggered dizziness teaches the brain to respond less intensely, helping symptoms fade over time.

When to Worry About Dizziness

It’s easy to brush off dizziness as “just being tired” or “getting older.” But persistent or worsening dizziness is not something to ignore. It can be a sign of an underlying vestibular disorder—especially if you also notice:

  • Imbalance or frequent falls

  • Blurred vision when moving your head

  • Feeling “off” in busy environments

  • Difficulty walking in the dark

  • Nausea with quick movements

If dizziness is interfering with daily life, it’s time to talk with a vestibular-trained physical therapist. The good news: with the right rehab approach, many people see dramatic improvements.
